    Living Well with Diabetes (Fremont)

    The Living Well with Diabetes small group series is for patients with type 2 diabetes who are either newly diagnosed or have not received diabetes education in a couple of years. 5 Sessions per class-dates listed are the starting date.

  • Understanding Diabetes and its risk factors
  • Blood glucose monitoring instruction
  • Interpretation of blood glucose patterns and making management decisions
  • Exercise and Nutrition guidelines
  • Blood pressure management
  • Diabetic emergencies and sick-day management
  • Preventing long-term complications
  • Foot care guidelines
  • Stress and diabetes
  • Insulin and oral medications
