Question
my g/f and I attempted to have sex. it was a total failure as I was nervous, and struggled to get the condom on. after that I could not get my penis in the vagina. I blame myself but she says she doesn't mind.
Answer
Relax. Anxiety and worry are the worst possible things you can do. The things you did right are talking and using a condom. Back up and take the decision and process slower. By yourself, practice putting condoms on until you are confident and competent. With your girlfriend, relax and keep talking - there are many things you can do to be intimate other than intercourse - get comfortable with each other and then things will go better.
