The Anticoagulation Clinic (ACC) at the Palo Alto Medical Foundation (PAMF) is a specialty clinic that focuses on the management of patients taking warfarin (Coumadin®). Warfarin is an anticoagulant medication used to prevent and treat blood clots. Its use requires careful and periodic monitoring, including regular blood tests, to ensure patients are being treated effectively and safely. Many factors, including other medications, diet, and alcohol use, can affect warfarin therapy.

The ACC is staffed by registered nurses, nurse practitioners, and pharmacists who all work together under a medical director. Unlike laboratory visits, the ACC providers perform finger stick blood tests to identify the level of warfarin in your body (the result is available in less than one minute) which is used to determine the appropriate warfarin dose for you. Clinic staff also assess for any changes in your medications and diet, as well as any side effects related to warfarin therapy.

To become a part of the ACC, your doctor must send a referral to the clinic. Some locations have a waiting list, but you will be contacted as soon as an appointment is available. Initial visits take about 30 minutes and subsequent visits are about 10 minutes.
