In 2006, PAMF's Patient-Focused Cancer Care Committee began to develop standards for survivorship care to support survivors while they are transitioning into normal life. The survivorship plan includes such post-treatment planning as psychosocial support (trained volunteers and support groups), shared decision making, nutrition support, a pain management plan and charitable care.

This section provides information about the programs available to cancer survivors. First, learn more about the Patient-Focused Cancer Care Committee.
