Cancer Survivors - Living Beyond Cancer
Because of newer, complex therapies, patients are living longer with their disease. Survivorship may be defined as the process of living with, through and beyond cancer. Living beyond cancer is an individual journey for each cancer patient and the PAMF Cancer Care team is committed to assisting patients to develop a wellness plan to monitor and maintain their health.
Often after treatment is completed, the question is raised: “what next”? The Palo Alto Medical Foundation Oncology Nurse Practitioners are available to patients for survivorship and wellness planning. The Nurse Practitioners work together with each patient to design a Living Beyond Cancer plan. This provides a roadmap to organize and keep track of their treatments, next steps, and other important information needed for coordinated care among primary and specialty physicians now and in future years. In addition, the nurse practitioners have resources available to them to assist with all aspects of care including nutritional counseling with registered dietitians, psychosocial support with oncology clinical social workers, complementary therapy programs, classes and community resources.
