For children 0 to 11: Complete and submit all well-child forms online through My Health Online''s child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you''ll avoid getting charged the usual form fee.
