Make things go more smoothly for your caregivers?
An advance directive is a statement of your wishes for the kind of treatment you would like to receive if you are unable to make medical decisions for yourself. It helps to lift the burden of decision-making from your family or caregivers.
Our specially trained volunteer will answer your questions and discuss issues that you may wish to consider. You will receive an Advance Health Care Directive form at this appointment, and you may complete it during the consultation. You can also print the form from our web site at pamf.org.
