Diabetes Appointments with Certified Diabetes Educators
Palo Alto Medical Foundation's (PAMF) Diabetes Education Department has a team of highly trained Certified Diabetes Educators (CDE) who provide education, coaching, counseling, and therapeutic assessment and intervention for all types of diabetes and associated disorders of carbohydrate metabolism. Patients may meet with a CDE individually as part of our comprehensive self management program.
The CDE addresses all areas of diabetes self management including nutrition, self monitoring of blood glucose, exercise, medication and stress reduction and coping. The CDE will instruct you on blood glucose testing and provide you with a glucometer. They will also train you (if needed) on the use of insulin, insulin pump therapy, and continuous glucose monitors. They will work closely with you to give you the tools to achieve optimal blood glucose levels through lifestyle management and medication when necessary. As part of the multidisciplinary team, CDEs work closely with your doctors to help implement your plan of care.
Individual consultation, medical nutrition therapy and diabetes self management education is available in many of our centers. Please talk with your doctor as you may need a referral.
