You can also download, print and complete the age appropriate well-child form
at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

Dr. Chow is married and enjoys diving, tennis, bicycling, playing the piano, and spending time with her family and friends. She also loves gymnastics and has won Olympic medals in the sport.

Philosophy of Care
I was born and raised in San Jose, California. A large part of my childhood included doing gymnastics and playing the piano. From these activities, I learned the importance of hard work and dedication. I valued being a team player and developed an understanding of how to approach challenges. I plan to use this same focus, enthusiasm and commitment to provide excellent care for your child.
One of my favorite aspects of pediatrics is the opportunity to develop long-term relationships with my patients and their families. I love seeing children grow up, and I am honored to be a part of their lives. I believe in the preventative approach to medicine, and I enjoy educating families so they can be active participants in their health care.
I am trained in all areas of general pediatrics but have a special interest in sports medicine. I think that the combination of smart training, injury prevention and effective rehabilitation techniques is especially important in young athletes. My goal is for your child to have a meaningful and fun sports career, as well as a long-lasting healthy lifestyle.
