For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

Philosophy of Care
First and foremost, I believe in the human capacity for positive change. Given encouragement and knowledge, all patients have the capability to improve, if not resolve, their medical conditions with minimal amount of medications. Although medication can be beneficial in some circumstances (infections, trauma, for example), I prefer not to prescribe any medicine due to potential side effects. All medications have potential side effects, no matter if they are prescribed or over the counter.
My role as a physician is not to give my patients a pill for every problem that they have. Instead, my role is to help my patients meet their goal for a healthier lifestyle by making sure they understand their medical conditions, encouraging better diet, promoting physical activities, and connecting patients to appropriate specialists and community resources.
