If you are expecting a baby, let us help you check a pediatrician's availability by submitting a newborn referral form. For children 0 to 11:
Submit all well-child forms online through My Health Online
's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form
. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
Dr. Chen does not accept new patients as a primary care physician but provides coverage for other pediatricians in the department.

Philosophy of Care
As one of the float physicians in our group, I cover for all the pediatricians when they are either fully booked or out of the office. Since each pediatrician varies somewhat in practice style, I cannot be like everyone else but I try my best to provide the highest standard of quality care. I strive to work on schedule in order to minimize wait time for our patients. I also enjoy educating the families and involving them in the decision-making of their health care.
