Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
