For children 0 to 11: Submit well-child forms online through My Health Online's child proxy access feature. Log into your My Health Online account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
For ages 65 and older: Complete a Annual Medicare Wellness Visit Questionnaire prior to your appointment.
Log into your My Health Online account. Fill out the questionnaire by clicking on the link in "Upcoming Appointments."

I want every patient to enjoy a long, healthy life, and everything I recommend is with this in mind. I strive to provide excellent care, and see my role as that of an advisor. I make recommendations based on what the patient tells me, their personal and family history and their examination. I try to provide choices and to help the patient understand the potential risks and benefits of different courses of action. I believe that health is more than the absence of disease and that taking time for friends, family, and fun is good for you. I practice minimal intervention.
