    If you are expecting a baby, let us help you check a pediatrician's availability by submitting a newborn referral form.

    For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

    As a pediatrician, I try to provide the best possible care for my patients while still showing respect and empathy for their situation. There is no more difficult challenge for a parent than caring for their sick child. My job is to do what I can to ease that burden for the parent and child. I am excited about being a part of an organization like PAMF that shares a commitment to the health of children.
