    For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

    Philosophy of Care

    I strive to provide high-quality care to my patients in a personal manner. When I am in the examining room with a patient, my mind is completely focused on my patient. I spend as much time as it takes to understand what their issues are before conducting an accurate physical examination that focuses on the issues at hand. After that, I engage my patients in discussions about their care and provide an opportunity for them to share in the decision-making process. I believe that rather than just telling my patients what to do, it is important to provide informed options so that they can choose what fits best with their personal values - to the extent that it is safe and reasonable to do so.

    I really enjoy just about all of the interactions I have with patients. I like explaining what is going on and helping them understand their health and medical conditions. I enjoy answering questions from my patients because I believe that the more patients understand, the better able they are to care for themselves. Most of all, I enjoy working together with my patients as a partner in their care.
