Save Time - Go Green! Complete Online Forms
For children 0 to 11: Submit well-child forms online through My Health Online's child proxy access feature. Log into your My Health Online account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
For ages 65 and older: Complete a Annual Medicare Wellness Visit Questionnaire prior to your appointment.
Log into your My Health Online account. Fill out the questionnaire by clicking on the link in "Upcoming Appointments."
