    For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

    Philosophy of Care

    Individual health often mirrors the collective health of our communities. Disease commonly results from human behavior and is preventable by community education and access to proper resources. The aspect of medicine that I find most exciting involves addressing social issues from a medical context. I believe that it is ineffective to treat human illness without also addressing the cultural or environmental conditions from which disease emerges. While this cannot be achieved in one office visit, it does happen with time.

    Medicine, for me, is a real interest in people. In primary care you truly enjoy your patients as complex and unique individuals. As a family practitioner I have the opportunity to know individuals in the context of their family or loved ones. Knowing the person, and not just their particular ailment, is as important to providing optimal care as is up-to-date medical information. Every patient is an individual with a new challenge. I enjoy individualizing my therapy in a way that is satisfying to both of us.
