If you're scheduled for a preventative health exam (i.e. physical/annual checkup) or to establish care, please read over the page -- how to prepare for your upcoming visit. It will save time for your discussion with your doctor by completing your medical history form and tests prior to your visit.
For ages 65 and older: Complete a Annual Medicare Wellness Visit Questionnaire prior to your appointment.
Log into your My Health Online account. Fill out the questionnaire by clicking on the link in "Upcoming Appointments."
