    Save Time - Go Green! Complete Well-Child Forms

    For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

I strive to address all of your concerns in a way that makes sense to you; to work together with you to prepare for the next steps in your child's development and health; to ensure that coming to the doctor is a positive experience for your child; and to cheer you on and let you know you are not alone if you sometimes wonder whether anyone else ever feels like parenting is even harder than expected!
