For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

Personal Interests
I am married to a strong and witty husband and have two above average, exceptionally charming children. I enjoy spending time studying 5th grade history and 8th grade science, as well as caring for our three cats, two bunnies and one snake. Gardening, mountain biking, and writing stories and poems fills up all three minutes of my weekly personal time. I also enjoy cooking, but not cleaning up after the party.

Philosophy of Care
I enjoy practicing a full range of family medicine, including care of the newborn, end-of-life care and everything along the way. I have a special interest and training in well-woman care and health maintenance. I feel strongly that caring for the entire family in the context of the community brings important perspective to health care. My approach includes patient education and a focus on setting appropriate goals for health maintenance and improvement.
