For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

I look forward to partnering with you in the care of your child. My goal is to deliver health care to your child with compassion and consideration. I am a firm believer in the power of preventative medicine, and will educate you about ways to keep your children healthy, so that they can grow, develop and thrive. I think medication should be used judiciously, but when your child is ill, I will offer all that is necessary to help your child recover. The advice I offer is based not only on my training and experience as a pediatrician, but also as a father of two children.
