's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account

You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
