    For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

    Philosophy of Care

    I love being a pediatrician. It is a such a unique privilege to take care of children through all the stages of childhood, and to watch with you as your child grows and matures over the years. I strive to partner with you and your child, and try to align my approach to care with your family's values as much as possible.

    Being a proud parent of two boys has added depth to my practice of medicine, and has given me empathy as well as lessons learned from my own experience of raising young children. I approach my patients as I would my own children. I make clear communication a priority, and I value speaking with my patients and families directly as much as possible. Educating patients and parents and guiding them through the sea of information that is out there gives me great satisfaction. Furthermore, I firmly believe in establishing a proper foundation for health from an early age and love helping families establish healthy habits.

    I have roots in the Bay Area, having done both college and residency training at Stanford. I am happy to be part of the community that I serve and look forward to meeting you and your little (and not so little) ones in Sunnyvale!
