Pediatric patients can be seen everyday of the year including evenings, weekends, and holidays. Evening, weekend, and holiday clinics are held at the Mountain View Campus in the department of Pediatrics and in the Pediatric Urgent Care.

For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
