For children 0 to 11: Submit well-child forms online through My Health Online's child proxy access feature. Log into your My Health Online account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.
For ages 65 and older: Complete a Annual Medicare Wellness Visit Questionnaire prior to your appointment.
Log into your My Health Online account. Fill out the questionnaire by clicking on the link in "Upcoming Appointments."

I strongly believe in a good line of communication with my patients. From this foundation, great things will happen. I really enjoy finding the root cause of your concerns and/or symptoms, so that we can address the issue at hand. I am a strong proponent of all modalities that may help you feel better, whether it's medication, dietary modifications, physical therapy, massage or counseling. I understand that each individual will be more partial to one kind of treatment or another and I aim to work with you to achieve the desired goal of good health.
I feel it's an honor and a privilege to have a role in the health of your family. I thoroughly enjoy seeing everyone grow and mature, ourselves included.
