New Service Features - Starting Dec. 1
From Dec. 1 you will be able to:
- Directly schedule your appointments online to see your primary care doctor.
- Submit online appointment requests for specialty physicians.
- Pay bills and co-payments and check your account balance quickly and easily online.
- Go to any PAMF laboratory or imaging facility for services that is convenient for you.

My Health Online - the Gateway to Many New Service Features
You'll have access to many of these new, convenient features through our online service My Health Online. If you are not yet enrolled in My Health Online, sign up today!
In addition to the new features described above, as a My Health Online member you'll also be able to:

Manage Your Child's Health Care More Conveniently Online, Too
My Health Online is not just a convenient way to manage your own health online, it's also an easy way to manage your child's medical needs. My Health Online's Proxy Access Program gives you full access to the health records of your children younger than 12. From anywhere in the world, you can:
- View and print vaccination records
- Complete questionnaires that track your child's development
- Review medication instructions and other information that can help you better manage your child's medical needs

Important Notice Related to Patient Billing Statements
Our patient statements will have a new look starting December 1. This is part or our transition to a new business system that allows us to provide new convenient features to our patients such as direct online appointment scheduling in primary care.
As a result of this change, there is a possibility that you will receive separate statements generated from both the previous system and the new one.
- If you have an outstanding balance for charges incurred prior to December 1, you will receive statements from the previous system until that balance is paid in full.
- Charges for services received after December 1 will be reflected in statements from the new system.
For services prior to December 1

For services December 1 and later

You Only Need to Make One Payment
If you receive separate statements, you only need to include one payment to cover your total balance. However, we ask that you also include the payment stubs that correspond with both statements to help ensure accurate and timely processing.
Individual Billing Makes It Easier to Understand Charges
We also will be changing from family billing, where families receive a single combined statement for multiple family members, to individual billing, where each patient receives a separate statement for multiple family members, to individual billing, where each patient receives a separate statement. This reduces any confusion that might occur when charges and payments for multiple patients are combined in a single statement.

We Need Your Signature Next Time You Come In
When you check in for your next appointment, you'll be handed a new Conditions of Registration form to sign. It explains in plain language what you should know before you can be treated, especially about your financial obligations for care. Typically, patients sign this form at their very first appointment.
However, we recently updated our form and are therefore asking all of our patients - new and existing - to complete the form. Thank you for your patience and understanding.
