Resetting Your Password for the Egg Donor Application Database

We have added new security measures to the PAMF Fertility egg donor application database to provide additional protection for your private information. This means that you will need to reset your password to log in to the database, even if you have used it before.


Please follow these simple steps to reset your password:

Click “Reset Password.” Your previous user name and password will no longer work.

Enter the email address you used when you originally registered for the program.


If your email address is in our system, you will receive an email with a link to reset your email. If our system cannot locate your email address, please contact PAMF’s Fertility Department at 800-597-2234 for assistance.

Below is an example of the email that you will receive if your email address is available to us. Please click on the link in the email to reset your password.

Choose a new password. Passwords must include both upper and lower case letters and one special character such as !, @, or #.

Enter your new password; then retype the new password to confirm it. Finally, click “Reset Password.”


You will be redirected to the page below. From there you can click to return to the login page and sign in. You should be able to access the progress of your egg donor application up until the transition to the new database system.
