Geriatric Subspecialty Consultation Services

This is an outpatient, referral-based program that serves PAMF patients over age 65. Our program specializes in the evaluation and consultative management of the functional, cognitive, and medical needs of community dwelling frail older adults.

Examples of types of referrals for assessments that we receive include:

• Memory impairment and impacts on patients and caregivers


• Recurring falls, fall risks, and needs for intervention


• Comprehensive geriatric assessment


• Advance care planning and goals of care discussions

We take referrals from primary care physicians, as well as subspecialty physicians throughout all PAMF divisions.
