Care in Skilled Nursing Facilities

PAMF geriatricians can help orchestrate direct on-site medical care and care coordination for PAMF patients at selected skilled nursing facilities.

We provide post-hospital care for medically complex patients who are not well enough to return home. We oversee post-operative patients who need additional medical and rehabilitative attention. We also guide care and direct advance care planning process for long-term care residents. Furthermore, we also work with community partners in providing onsite palliative and hospice care.

Our team members also work with some patients residing in assisted living and memory care units, through selected continuing care retirement communities.
