    Services

    As part of the requirement for obtaining entry and/or residency to the United States, a medical examination is necessary as part of the application process. The Occupational Medicine/Travel Department offers INS examinations conducted by a civil surgeon who is responsible for the entire examination and will record the results on the required forms.

    INS medical examinations are conducted bt appointment or on a walk-in basis. The examination will last approximately one to two hours.

    The medical examination is required for persons of all ages. The examination requirements are determined by your age and your immunization record. An adult examination (15 years or older) typically consists of the following:

    • A physical examination
    • A tuberculin (TB) ) test is required for all persons 2 years or older. If you have tested positive in the past you must provide written documentation otherwise repeat testing is required. A chest x-ray within 6 months of the examination is required for anyone with a positive test.
    • A blood test for syphilis (RPR)
    • Routine immunizations for your age group is required
    • A pediatric examination (children 14 years of age or younger) does not require blood testing and typically consists of the following:
      • A physical examination
      • A TB test
      • A review and update of routine pediatric immunizations.
    To better serve you and your family members, please bring the following items with you on the day of your examination:
    • INS Form I-693
    • Supplemental Form To I-693
    • All medications and immunization records for each family member seeking entry, especially important for pre-school and school-age children
    • Any recent records of blood tests for AIDS, syphilis, or measles (rubeola), rubella (German measles), mumps, or chickenpox titers
    • Any report of a chest x-ray (within 6 months)
    • Any records of a test for TB
    If you are found to have a health condition that requires medical follow-up, medical clearance will not be granted until follow-up is completed.
