 | To guarantee proper collection, the phlebotomist is required to verbally confirm the patient's name and date of birth. |
 | To assure patient safety -- a new and unused needle kit and tubes are shown to the patient prior to the blood being drawn. |
 | The labeled and initialed tube is checked against the requisition for accuracy. |
