Menlo Park Surgical Hospital (MPSH) nurses will contact you before your surgery to complete a preoperative assessment. If you have not been reached by the day prior to your surgery, please call (650) 324-8500 and ask to speak to a nurse regarding your preoperative assessment.

You will also be contacted by our registration staff to complete admitting and insurance information.

In this section you can review the forms that will be given to you upon admission. Although you will need to sign them in person, please take the time to review all the information in advance.
