Patient Information
Patient-Centered Care
To improve each patient's experience, our office visits are based on a patient-centered care model. This new approach allows you to spend more quality time with your health care providers and less time waiting.
When you arrive, our medical assistants (MA) greet you and immediately escort you to an exam room where your medical history is taken and your electronic health record (EHR) is updated. The MA continually checks in with the physician and remains with you throughout your visit. After you meet with your provider, the MA makes follow-up appointments for you, if needed.
With our patient-centered model, you benefit from a personal, attentive approach to health care.
Referral to Specialists
The physicians of Castro Valley Primary Care have access to a vast network of specialty care physicians. If you have a medical concern that requires a specialist, schedule an appointment with your primary care provider. If necessary, your physician will refer you to local specialist or a PAMF physician.
Prescription Medication Refills
To request a refill, call the pharmacy that dispensed your prescription. If you have a specific question about a refill that cannot be addressed by the pharmacy, call Castro Valley Primary Care.
Laboratory and Radiology Services
Some laboratory procedures, such as blood draws, can be done on site. Radiology services are available at Eden Medical Center. If X-rays are necessary, your primary care physician will refer you to Eden's Radiology Department.
