Special note for Medicare Advantage Plans

PAMF currently participates in the following Medicare Advantage Plans:
  • Anthem Blue Cross Freedom Blue Regional PPO (RPPO)
  • Health Net Seniority Plus (HMO)
  • Secure Horizon’s (HMO) – For patients seen in our Cupertino, Mountain View, Santa Clara, Sunnyvale and Santa Cruz County locations.
PAMF also participates in hundreds of Medicare Supplement (or "Medi-Gap") insurance plans. However, at this time, PAMF does not participate in Medicare Advantage Private Fee-For-Service (PFFS) plans. The open enrollment period for Medicare Advantage plans is November 15 to December 31. For more information, visit www.medicare.gov, or call 1-800-MEDICARE (1-800-633-4227).

Additional Information

  • Your health plan may have a specific process for seeing a specialist. Many health plans require your primary care physician to refer you to a specialist, while others allow you to self-refer. Before making an appointment with a specialist, please check with your health plan to ensure the visit will be covered.

  • We make every effort to keep this list as accurate as we can for our larger health plans. Even so, it is possible that not every health plan and circumstance is addressed here. You can obtain the most accurate and timely information by calling your health plan or employer benefits department and confirming your coverage level when choosing a PAMF physician.

  • Some examples of health plans for which we are not a preferred provider are: BCE/Emergis, CCN, and PHCS. This list is not intended to be all-inclusive.

  • The employer list can change at any time, patients should confirm with their employer's administrator if PAMF is a preferred provider for their employer.
