    How does one make an appointment for physical therapy?

    You will first need to obtain a referral from your doctor. You can then call the office at 831-458-6230 to schedule your appointment.

    What should I expect from my first appointment?

    Please arrive at least five minutes prior to your scheduled time to allow time for checking in. At your appointment, a licensed rehabilitation therapist will review your doctor's notes and instructions, and complete an evaluation of your problem. The therapist will then design an individualized program to help you achieve your goals. This plan will be carried out by our team of experienced therapists and therapy assistants.

    How do I get to my appointment, and where should I park?

    We are located at 1529 Seabright Avenue, near the corner of Soquel Avenue in Santa Cruz. The entrance to the parking lot is across from Gault Street. Our clinic is in the second building from the street. There is also parking in front of our facility.

    How long will my appointment be?

    The typical therapy appointment is scheduled for 30 minutes, but it may run longer.

    What types of insurance does the Physical Therapy Department accept?

    PAMF provides physical therapy services to patients with HMO, POS and PPO insurance for whom our physical therapy services are appropriate. HMO and POS patients are required to have a referral and authorization from their PAMF primary care physician prior to beginning physical therapy.

    Note: PAMF is currently completing the certification necessary to become a Medicare provider for physical therapy. Patients should verify that services are covered by their insurance provider prior to receiving services.
