Toxoplasma Serology Lab Requisition Forms

There are four different requisition forms, each for a defined group of patients requiring toxoplasmosis testing.

All forms are Acrobat files:

1) Testing in Pregnant Women

2) Testing in Newborns and Infants
(1 year of age or younger)

3) Testing in Non-Pregnant Adults and Older Children (more than 1 year of age)

4) Immunocompromised Patient

In general, these "Recommended Tests" will provide the best combination of test results for your patient's specific needs. In addition, providing us with the requested clinical information about your patient is critical for interpretation of test results. Following the algorithm of recommended tests and giving us access to your patient's clinical scenario will enhance our ability to properly interpret your patient's test results in a timely and efficient manner.

Please note that we will continue to offer individual tests and panels.
